Allergen Information
In order to ensure that Oroville School District's Food Service is aware of your child's dietary restrictions, such as food allergies or diabetes, the parent/guardian will need to complete a request for Special Dietary Accommodations form. This form is required to be signed by a State Recognized Medical Authority and must be submitted to the School Nurse. If the district is aware of a meal modification, the district will not unduly delay providing the modification while awaiting the medical statement. Instead, the district will begin to provide reasonable modifications to keep the child safe, and request the family provide a signed medical statement to support the meal modifications as soon as possible.

CIVIL RIGHTS COMPLAINTS OF DISCRIMINATION PROCEDURE AND FORM
Complaints of discrimination may be written or verbal and filed within 180 days of the alleged discrimination. Use of a form is not required for a person filing a complaint. If a person is unwilling, unable, or not inclined to put the complaint in writing,the person taking the complaint shall do so.
